Doctor Sinitsyna: Not sugar life of diabetics

– Elena Nikolaevna, about you speak as about the high-class professional. What deserved such success?

– To be successful in our business, it is necessary to be the fanatic. Here I in some sense the fanatic of the business also worked always where conditions for my improvement were provided.

I call myself "the soldier of medicine" because I passed serious school: three years in rural areas on ambulance, then in the central district hospital, then in the 67th city hospital, and then I got to hospital for veterans of wars – very good, smartly equipped where worked 16 years.

I went to this hospital consciously to grow in the profession. There was a hormonal laboratory, nearly only at that time in Moscow, and I could use all opportunities of modern diagnostics, check the level of hormones on the special equipment and the nuclear magnetic resonance then still was a new experience. At department where I studied, for all its history did not make so many diagnoses of rare diseases how many I put in hospital, having an opportunity to examine people on the modern equipment.

The fact that in this hospital there were such difficult patients who owing to age had the whole complex of pathologies allowed me to promote as the expert. And in our narrow medical world if you become the authoritative expert, then colleagues always know about it.

– From where do you have interest in diabetes why to it?

– Likely, because at the heart of a metabolism – my favourite chemistry, it always came to me easily. At school we had a class with profound studying of this subject. The medical institute learns the first two years of future doctor to chemistry, physics, and it seems to all that it is so far from medicine. And I consider that the profound knowledge of these sciences and all laws of physiology, the doctor is better. How there glucose do interact with insulin? Main question of diabetology, continuous theory. When well you understand a subject, there is a wish to understand even better, even more deeply. It carries away.

– Therefore you also came to endocrinology?

– It was the conscious choice. Working as the emergency doctor, I became interested in cardiology. And there saw that five heart attacks come to the cardioblock in a night, three from them are diabetics, and nobody is able to treat them, and even the most experienced cardioresuscitators are often lost at the high content of sugar in blood. All algorithm of treatment changes. Solved: it is necessary to become the endocrinologist. It is impossible to treat!

After an internship I for professional development was directed to work in resuscitation. At that time in the 67th hospital there was just very high mortality from diabetic a lump – higher than 20%. And I lowered it to 6%.

There, in resuscitation, I in a year cured about 200 diabetic whom. Without boasting, you will seldom find the endocrinologist who would bring so many diabetics out of a coma as I. Having seen my results, the chief endocrinologist of Moscow of that time Alexander Mazowiecki wanted even to open separate endocrinological resuscitation. But it appeared to create too expensive separate resuscitation for diabetics, and the idea buried. Diabetics in this sense are not lucky at all …

– Why?

– Insurance companies are not engaged in diabetes. To insure the diabetic – huge expenses. Are insured on a case of such diseases which either recover, or demand insignificant time and financial expenditure, and diabetes does not concern those.

It turns out, the patient rushes about itself – depending on as far as he is anxious with the health. He is provided to himself and cannot penetrate into all subtleties of the disease. And it is not enough, it is not enough specialists endocrinologists who can effectively treat and are not indifferent to the fate of the patient.

Today my specialization was the leader, diabetes in many respects – time disease. I constantly have a work for a brain, and the accumulated knowledge is very demanded. Unfortunately, diabetes are not able to treat anywhere: neither in America, nor in Europe. There in general was a paradoxical situation: it is claimed that diabetes not a disease, but a way of life. I had patients from everywhere. I only am surprised to inefficiency of their treatment.

– And how it is possible to estimate efficiency of therapy of obviously incurable disease?

– According to the international standards evaluation criteria is the glikirovanny (candied) hemoglobin, cholesterol, excess weight. The healthy person has a level of glikirovanny hemoglobin in blood – 6%, at diabetics it can be twice higher depending on extent of compensation of a disease. To normalize this indicator very difficult. Around the world it is considered good result to reduce its level at least by 1% within a year. At vast majority of patients of our clinic it decreases by 1.5-2.7% in 21 days of treatment.
