LASER PEELING of the FACE. FRACTIONAL PEELING. Article No. 2

One of the most popular techniques of rejuvenation in modern cosmetology – a laser peeling of a face. It is option of physical clarification on the basis of monochrome light radiation. The wave of one length makes impact on fabrics, leading to their selective heating. This reaction is resulted by the evaporation of certain layers of skin leading to removal of pigmentary spots, acne rash, mimic wrinkles and other esthetic defects.

For achievement of the rejuvenating and therapeutic effects two options of influence can be used:

Hot peeling. For holding a procedure the carbonic laser which is capable to subject the processed fabrics to considerable heating is used. Such technique increases risk of development of burns and increases the period of restoration of epidermis. With extra care processing of area of a neck and a century is carried out. First of all such laser peeling of a face is used for disposal of extensions, hems, new growths and grinding of an integument. At an extreme power the carbonic beam can cut fabrics. MECHANISM OF IMPACT OF THE LASER ON FACE SKIN
Thanks to high precision of modern devices for a laser peeling exact control of depth and intensity of influence is provided. It allows to delete evenly and effectively horny cages, to start the mechanism of regeneration and restoration of fabrics.

Allocate three extents of influence with the laser for skin:

- Laser surgery of cicatricial fabric.

- Deep.

- Superficial.

The laser peeling of a face belongs to this group. It does not damage integrity of a basal membrane and can make cold and hot impact. Here it is possible to refer also esthetic grinding of an integument. Its difference from a peeling of a similar type consists in penetration depth. The laser peeling of a face is recommended to be carried out at the following symptoms and states:

- pigmentary spots, enlarged pores, superficial wrinkles, superficial hems;
- decrease in a tone and elasticity of an integument;
- thickening of a horn layer of epidermis;
- follicular cysts;
- extensions.

The list of contraindications to holding a session quite impressive also includes the following states:

- pustulous or viral rash, warts, a contagious mollusk, bacterial and pustulous diseases on - a working surface;
- the blood diseases reducing the speed of its folding;
- cardiovascular, autoimmune and oncological diseases;
- the cold and any states leading to temperature increase of a body;
- immunodeficiency, diabetes;
- diseases of connecting fabric, tendency to formation of hems;
- existence of tattoos and fresh suntan on a surface which is subject to processing;
- presence of fillers under the processed site;
- birthmarks or birthmarks;
- swarty skin (the type and degree of risk are defined by the expert);
- period of pregnancy and lactation;
- epilepsy, a stroke and tuberculosis in the anamnesis;
- the established pacemaker;
- psoriasis and period of aggravation of a number of skin diseases;
- reception of a number of medicines;
- intolerance of anesthetics;
- violation of integrity of epidermis, including as a result of a chemical peeling.


Pluses and minuses of a laser peeling

The main pluses of the procedure are obvious when performing cold face peel by means of the laser:

- Due to instant evaporation of liquid from fabrics there is no heating of deep skin layers therefore the burn of skin is excluded.

- Even at repeated influence by the laser on the same site of epidermis, the necrosis of fabrics does not occur. It significantly reduces risk of development of complications and side effects.

- The laser peeling of a face by any method provides the lasting rejuvenating and cleaning effect.

It is possible to distinguish its quite high cost and insignificant depth of influence from shortcomings of a technique. In such a way not to remove deeply lying pigmentary spots and the expressed wrinkles. In addition, manipulation can be followed by pain and discomfort.

MAIN TECHNIQUES OF CARRYING OUT LASER PEELING

Today cosmetologists allocate two main techniques which presented a cosmetology laser peeling of a face:

- Traditional option of influence which means capture of all surface of the processed zone which is in limits of a laser spot.

- Fractional peeling. This equipment means rendering therapeutic action on small sites in the processed area. Usually it is no more than 20-25% of a surface.

When choosing an optimal variant of impact on epidermis it is necessary to consider the fact that the fractional peeling has the list of synonyms and you should not address experts who tell about the rejuvenation options given below, as about different procedures:

- DOT or DOT-therapy;
- drilling;
- seamless laser tightening;
- fractional grinding by the laser;

- laser rejuvenation on frakselny type;
- fractional remodeling of skin.

The fractional peeling is the most widespread method of impact on skin by means of the laser. The essence of a technique consists in special distribution of energy and creation of the medical effect leading to the accelerated formation of collagen and elastin in the course of formation of new fabrics. Such result is achieved at the expense of the accelerated cell fission near the damaged zones.

Researches of scientists showed that the fractional peeling leads to consequences as a result of which some cages perish, and others – rejuvenate. Those structures which are not capable to regenerate any more perish. Thus, there is a replacement of the fulfilled material with new colonies.

The fractional peeling creates a peculiar thermal shock which stimulates activity of cages and starts processes of updating of epidermis. The gained effect is affected by characteristics of the used laser beam and depth of its influence. Thanks to development of modern technologies, cosmetologists are capable to influence with a dot clearness microscopic zones, forming the injured tubules in an environment of untouched fabric.


Carry out a fractional peeling in the presence of the following indications:

- extensions, scars and hems of various etiology;
- roughnesses on the surface of epidermis;
- small wrinkles, "goose pads";
- age superficial pigmentation and multiple freckles;
- flabbiness of fabrics;
- unhealthy complexion.

Most often the fractional peeling is carried out by a technique under the name fraksel. It is the special patented version of the cosmetology procedure which is applied on devices of one type and has a number of advantages. Existence of a control system on the basis of a set of sensors allows to control quantity of burns on unit of area, selecting the optimum density of impulses.

The fractional peeling strictly on doctor's orders is carried out, the number of procedures is selected individually.


TECHNOLOGY OF CARRYING OUT THE PEELING BY MEANS OF THE LASER

How to carry out a laser peeling of a face it is necessary to prepare skin. The surface of a face is carefully cleaned with soft gel and processed by the calming lotion. Then local anesthesia is carried out. It is necessary to remember that if at the patient allergic reaction to lidocaine derivatives was noted earlier, it is necessary to take in advance the antihistaminic drugs which will be appointed by the cosmetologist. The last stage of preparation – putting special anesthetic of cream under occlusion. In 30-60 minutes weight is deleted, and the integument is processed by lotion.

Before beginning the procedure the doctor puts on goggles on himself and the patient. The laser peeling of a face occupies no more than half an hour and provides influence by a laser beam on problem sites of skin. Thermal damages of the required degree lead to the accelerated epitelization and reduces risk of development of side effects.

Depending on type of epidermis and the required result the expert can control the power of a power stream, the size of a laser spot, duration and the nature of an impulse.

When all manipulations are ended, the surface of skin is processed by the calming cream, sometimes use special lotions. Then epidermis is moistened. Very seldom after manipulation hypostasis of fabrics, face reddening, peeling and feeling of discomfort can be noted. The phenomena pass within two days. It is possible to apply cold compresses and water-based creams to acceleration of process.

The laser peeling of a face does not provide long and special care of epidermis. Already next day after the session it is allowed to apply the decorative and looking after cosmetics. Use of sunblock cream is in addition recommended.


POSSIBLE COMPLICATIONS AFTER THE PROCEDURE

Even at the correct holding a procedure the laser peeling of a face can cause development of specific consequences and complications:

1. Bubbles and erosion in the place of their opening. It is result of a thermal burn. Educations need special leaving, otherwise there can be a formation of hems and zones of a hyperpegmentation.

2. Dot hemorrhages. If the laser peeling of a face damages small vessels, blood streams on a surface small droplets, forming bruises. The state passes independently, without special leaving.

3. Effect of a gauze. The phenomenon represents a congestion of a large number of small atrophic hems. It results from individual tendency of epidermis to formation of scars or the wrong setup of the device. For installation of optimum indicators it is necessary to determine previously thickness of epidermis and terms at the patient. For this purpose use ultrasonic scanning or a histologic research of skin.

4. Hypopigmentation. If the laser peeling of a face led to focal death of melanotsit, then on skin roundish light spots which will not be subject to suntan are formed.

5. Puffiness of fabrics. It is normal reaction of an organism to influence which can be excessively expressed.

6. Post-inflammatory excessive pigmentation. Meets quite often, badly moves to treatment.

7. Exacerbation of acne rash. It is observed as a result of decrease in barrier function of epidermis, treated by profile medicines.

8. Spread of warts and a herpetic infection on earlier not occupied zones. Scientists did not prove yet that it is possible to delete warts by means of the laser therefore for their treatment use liquid nitrogen. Virus manifestations are removed intake of medicines.
